Terms of Service

Fage is a licensed insurance agency. We are not an insurance carrier, a third-party administrator or a health plan. We present plans offered by carriers, associations and administrators, and we help you apply to them. Coverage is issued by the carrier or plan, not by us, and is governed by that plan’s own documents.

Rates shown on this site are estimates based on the information given and the rates published to us at the time. They are not an offer of coverage and they are not a guarantee of price. A final rate, and whether an application is accepted at all, is determined by the carrier or plan administrator. Some plans are medically underwritten and may be declined.

Enrollment cut-off dates, effective dates and waiting periods are set by each plan and can change without notice to us.

You agree that the information you submit is accurate and is yours to submit, or that you are authorized to submit it for a member of your household or your employees. Applications carry legal weight: a material misstatement can void coverage or a claim.

Independent agents appointed with Fage may present these plans on their own pages. An agent is responsible for their own advice and conduct. Nothing on an agent’s page changes the plan terms set by the carrier.

Where you sign an application electronically, you agree that your electronic signature has the same effect as a handwritten one and that we may deliver plan documents, confirmations and notices to you electronically.

Nothing on this site is medical advice. Drug lists, provider directories and benefit summaries are supplied by carriers and may be out of date. Confirm any drug, doctor or benefit with the plan before you rely on it.

Fage Financial Services operates two text message programs. You may be enrolled in either, both or neither.

Account and service messages. If you give us your mobile number in connection with an application or a policy, we may text you about that application or policy — signature requests, a payment that did not go through, a document we need, a question from your agent, and similar. Message frequency varies with the activity on your account.

News and offers. Separately, and only if you have agreed to it, we may text you about new plans, open enrollment and other offers. Message frequency varies and will not normally exceed a few messages a month. You can be on our service messages without being on this one.

Message and data rates may apply. Consent to receive text messages, service or marketing, is not a condition of purchasing any goods or services.

To stop. Reply STOP to any message to be removed from that program. You will receive one confirmation and then nothing further from that number. Because our service messages and our news and offers are sent from different numbers, stopping one does not stop the other — reply STOP to both if you want no text messages from us at all. Please note that stopping service messages means you will not be told by text about a failed payment, a signature we need or a document that is missing. Reply START to the same number at any time to begin receiving messages again.

Carriers are not liable for delayed or undelivered messages. Supported carriers may change without notice.

How we handle the mobile number itself is set out in our Privacy Policy. In short: we do not sell it and we do not share it with anyone for their own marketing.

The design, text and tools on this site belong to Fage or to the platform that operates it, except for carrier materials, which belong to those carriers. Do not copy or republish them without permission.

The site is provided as is. To the fullest extent the law allows, Fage is not liable for indirect or consequential loss arising from use of this site or reliance on information published on it. Nothing here limits any liability that cannot lawfully be limited, and nothing here affects your rights against a carrier under a plan you hold.

We may update these terms. The date at the top says when they last changed. Material changes to the text message program will be described in a message to anyone enrolled in it.

These terms are governed by the laws of the State of New York, without regard to its conflict of laws rules.
