You pay $25 for primary care visits and $50 for specialist or urgent care visits, with telemedicine available at no cost. Preventive care — including annual wellness exams, immunizations, and a wide range of screenings — is covered at 100% with no cost to you. For prescription drugs, the plan includes tiered coverage: preventive generic medications are free, while other preferred drugs range from $15 or less up to $60 or more depending on the tier, with an individual monthly prescription benefit maximum of $250. The plan also covers basic lab work, radiology, and major imaging like CT and MRI scans in-office, each subject to a per-visit copay and annual limits. If you need hospital care, inpatient services are covered at 50% coinsurance and are limited to 5 days per year, while inpatient and outpatient surgeries are also subject to annual limits.
