Routine office visits are straightforward — primary care visits cost $45, specialist visits are $90, and urgent care is also $90. Preventive care is covered at no cost when you see an in-network provider. Telemedicine is included through Evo at no additional copay, giving you convenient access to care from home. For bigger medical needs, the plan has a $3,500 individual deductible. Once that is met, you pay 20% of covered costs (coinsurance) and the plan pays 80%, until your total out-of-pocket costs reach $7,350 for the year — at which point the plan covers 100% of eligible in-network expenses. Hospital stays, surgeries, emergency room visits, imaging like CT and MRI scans, and serious treatments like chemotherapy and dialysis are all covered after the deductible. Note that many hospital and facility services require prior authorization, so it is important to check before scheduling procedures. Prescription drug coverage is included with competitive copays: preventive generics are free, standard generics are $15 for a 30-day supply, preferred brand-name drugs are $65, and non-preferred brands are $100.
