HSA 8050
PHCS / Multiplan Network
The HSA 8050 is a high-deductible PPO plan designed for generally healthy, working adults who want lower monthly premiums and the ability to save money tax-free through a Health Savings Account (HSA). Once you meet the $8,050 individual deductible, the plan pays 100% of covered in-network services for the rest of the year. It's best suited for people who don't expect frequent medical care and want to build long-term healthcare savings.

The HSA 8050 plan is a high-deductible PPO that gives you access to the broad PHCS/Multiplan national network in all 50 states, with no referrals needed to see specialists. The plan has an $8,050 individual deductible (and $16,100 for families), which means you pay for most covered services out of pocket until that threshold is reached. Once you've met the deductible, the plan covers 100% of in-network costs for the rest of the year — including doctor visits, specialist care, hospital stays, surgery, mental health services, pregnancy and delivery, and prescription drugs. Preventive care, such as annual checkups and screenings, is covered at no cost before the deductible. Telemedicine is also available at no cost to you. Because this is an IRS-qualified High Deductible Health Plan (HDHP), it comes paired with a Lively Health Savings Account (HSA). The plan includes a $25 per month contribution toward your HSA, and you can contribute up to $4,300 (individual) or $8,550 (family) in 2025 on your own, tax-free. HSA funds can be used to pay for a wide range of qualified medical expenses, including doctor visits, prescriptions, dental, vision, and more — and the money rolls over year to year, so it builds over time. This plan also includes $20,000 in life insurance through Humana for the primary member at no additional cost. Prescription drugs are covered after the deductible for non-preventive medications, and preventive generic drugs are covered at $0 with no deductible required. One important pharmacy note: all prescriptions are subject to a $200 per prescription cap — amounts above $200 are not covered by the plan. Specialty drugs require prior authorization. Out-of-network care is available but comes at significantly higher cost, so staying in-network is strongly recommended. This plan is a strong fit for self-employed individuals or association members who are generally healthy, want to keep monthly premiums lower, and are comfortable covering routine costs out of pocket while building tax-advantaged savings. Keep in mind that you must be actively working to qualify, and many services beyond primary care require prior authorization — failure to get prior authorization can result in a 50% reduction in benefits. Services not covered include dental, vision, acupuncture, skilled nursing facilities, infertility treatments, and care received outside the U.S.
