There is no deductible, no annual dollar maximum on dental, and no waiting period — coverage begins on your first day and pre-existing conditions are not excluded, as long as the procedures are listed on the plan's covered services schedule. Enrollment is guaranteed, meaning no health questions are required to join. On the dental side, preventive care — including cleanings (up to two per year), routine x-rays, fluoride treatments, sealants, and oral evaluations — is covered at $0 out of pocket. Basic services like fillings (both tooth-colored and silver) have modest fixed copays, and major services including crowns, bridges, dentures, root canals, periodontal treatment, implants, and orthodontics (for both children and adults) are all covered at set patient charges listed in the plan's Patient Charge Schedule. Because this is a DHMO plan, you must choose a network general dentist who manages your care; referrals are required for most specialty services, though pediatric dentists, orthodontists, and endodontists can be seen without a referral. The vision benefit, provided through GVS, covers a comprehensive eye exam every 12 months at no charge, plus eyeglasses or contact lenses. Frame coverage includes a $150 allowance toward non-collection frames or full coverage for frames from the GVS collection, with a $45 copay for the eyewear visit. Single vision, bifocal, trifocal, and progressive lenses are covered, along with a range of coatings.
